- Respiratory therapist evaluates patient and meets with care team
- Determining your insurance coverage and co-pay (Medicare, Medicaid and major insurance accepted)
- Home safety assessment
- Daily patient and caregiver education with equipment training prior to hospital discharge
- Setup of equipment in patient’s home
- Respiratory therapist present during patient home transition
- Ongoing follow-up and support, preventing rehospitalization
